Are you an experienced Adult Social Care Administrator looking for an opportunity to develop your career?
Then we would love to hear from you!
Our Adult Social Care Reablement Service is seeking a committed, compassionate, and skilled Referral Taker (Grade 5) to join our Gloucester-based countywide team. This is an excellent opportunity to be part of a forward-thinking service that places people at the centre of everything we do.
This opportunity is embedded within our reablement offer, applying strengths-based approaches and functional interventions to support people to “do for themselves” and achieve sustainable, independent outcomes. This role forms part of our ongoing service development, and the successful candidate will join a supportive and innovative environment where professional development is actively encouraged, and your contribution will have a meaningful impact.
About The Team/service
Our team plays a key role in supporting adults across Gloucestershire who have care and support needs as a result of illness, disability, or changing life circumstances.
You will support our team to work across a range of settings, including community-based teams, locality services, hospital discharge pathways, and specialist services, supporting individuals to overcome barriers and maintain independence in their daily lives.
Our approach focuses on working in partnership with individuals to understand their goals and priorities, enabling them to achieve outcomes through practical interventions and effective support. This is a dynamic and evolving service, with a strong emphasis on collaboration, innovation, and continuous improvement.
If you share our values and are committed to delivering high-quality support that promotes independence, dignity, and positive outcomes, we will welcome your application.

About The Role
The Referral Taker role is the first point of contact in the team; responsible for triaging information provided, liaising with partner agencies and taking comprehensive, holistic referrals.
The role is part of the Administration Team, and you can be confident of joining a welcoming, supportive, and experienced team. The post is based in Gloucester, and we offer flexible working with a mix of office and home working.
About you:
The successful candidate will have excellent communication skills and the ability to remain calm and focused in a busy environment, maintaining a positive team working approach. The role needs a holistic approach and an ability to create accurate referrals of appropriate urgency for relevant teams or for signposting and directing to other agencies.
You will have strong organisational skills with experience of responding flexibly to changing priorities. Experience of working in a customer facing environment is essential, as are excellent IT skills. Knowledge of the Liquid Logic (LAS) database would be beneficial but full training will be given.
Our values are at the heart of everything we do and how we treat each other. They provide essential guiding principles about the way that we work and set the tone for our culture, and identify what we, as an organisation, care about. See more about our Values on our website.
If you hold the same behaviours and approach, then we want to hear from you! In exchange, we will provide you with full training and support to help you succeed in your role.
